The ADFACT Community Team operates a range of programs for the ACT community, including The Nexus Program (incorporating the Men’s Halfway House Program and Outreach Program), Relapse Prevention, the Sober Driver Program, the Personal Support Program (PSP) and Workforce Development and Training. The Community Team focus is on programs delivered in community-based settings. Our objective for all programs under this umbrella is for:
The Nexus Program offers three to six months residential treatment including assessment, education, support and referral. It is the 'linking phase' between treatment and re-integration into the community. It's a time of preparation and the consolidation of knowledge and skills gained in earlier stages. The Nexus Program accepts referrals directly from the community, from Karralika and from other programs (including other residential therapeutic communities interstate). The Nexus Program offers community-based, through care providing clients with a supportive and gradual community re-entry phase – moving through voluntary work and training into full-time employment or education. Target Group Men and women who have completed a treatment program for alcohol and/or drug problems. The Nexus Program is able to accept clients who are utilising replacement pharmacotherapy treatments and who are committed to ongoing treatment. This may include maintenance treatment, or withdrawal, to abstinence. Admission Criteria Clients must be:

Program Components The Nexus Program sees people begin to take their place back in society. As clients move from the community or the protected environment of residential programs to a Nexus program house and then into their own accommodation in the community, they are supported by weekly case management and group sessions, as well as counselling/support sessions with Community Team staff. Attendance at Narcotics Anonymous or other similar community-based self-help groups also continues during this stage of the program. Associations established with other agencies for a range of services to ensure the most successful transition possible in the wider community also continue. Particular emphasis at this stage is placed on career development, social relationships and practical living skills, which are supported by innovative partnerships with other community agencies.
